THERE ARE THREE DIFFERENT WAYS YOU CAN CONNECT WITH ONE OF OUR HEALTHCARE PROVIDERS OR CANNABINOID THERAPY EDUCATORS AT NATIONAL ACCESS CANNABIS MEDICAL (NACM):
- By Medical Document: If your physician or nurse practitioner is willing to prescribe medical cannabis, they can simply fill out our MEDICAL DOCUMENT (PRESCRIPTION) and fax it to the number located in the top left corner of the form. NACM’s Cannabinoid Educators and Clinic Assistants will then contact you within 24-48 hours to guide you through the process and provide education.
- By Referral Form: If your own physician or nurse practitioner does not feel comfortable prescribing medical cannabis, they can refer you to one of our NACM clinics, where you will be assessed by an NACM healthcare practitioner, either in person or by telemedicine (note: telemedicine is not available in Alberta). To make a referral, your physician or nurse practitioner simply fills out the NACM REFERAL FORM and faxes it to the number located in the top left corner of the form. NACM staff will then contact you within 24-48 hours to guide you through the rest of the process.
- By Medical Information Release Form: If you do not currently have your own physician or nurse practitioner or are facing difficulties receiving a referral to our clinics from your own physician or nurse practitioner, please fill out our MEDICAL INFORMATION RELEASE FORM and fax it to the number located at the bottom of the form or scan and email it to the email address that appears at the bottom of the form. A Cannabinoid Therapy Educator or Clinic Assistant will then get in touch with you, within 24-48 hours, to discuss next steps.
